Primary Care Team of the Year finalists' showcase
This award recognises GP practices that demonstrate improved outcomes in patient care
Category sponsored by:
Broadmead Medical Centre, Bristol, UK
Over
the last year, Broadmead Medical Centre has been running Primary Care
services to young people in Central Bristol who may not have access to
GP surgeries. With high levels of risk in drug and alcohol misuse and,
in sexual behaviour, they frequently present late to emergency services.
The team at Broadmead medical centre, including medical, nursing, and
administrative staff, together with a drug support worker focused on
sexual health and contraception in weekly visits. So far, they have
enrolled 113 patients who were not previously registered and had great
results.
University of Nottingham Health Service (UNHS), UK
With
an expanding population in recent years, the UNHS identified mental
health issues in students as a clinical priority. Bringing in new
expertise, a consultant psychiatrist began a weekly clinic in the
surgery, increasing the provision of psychological services and hours of
a mental health advisor. They developed a new emotions and eating
clinic and created a research post, funded by the PCT, to study the
health needs of international students.

Rainbow Surgery, Cambridgeshire, UK
A
partnership between the Rainbow surgery and the Gainsborough Foundation
lead to promising results in detoxification of those seeking help with
alcohol problems, most of whom receive this within 48 hours. Of one
hundred patients seen in 30 months, 68% remained sober at 6 months and
60% at one year. This network also helps patients to rebuild their
social lives. Because of its success, the project has now expanded to
include other local surgeries.
